Complex Needs Carein the community

At LV Care Group, we specialise in supporting individuals with complex needs to live safely, comfortably and with dignity in the community. Our complex needs service is designed for people who require a higher level of support, but who want to remain in their own home and maintain as much independence as possible. This is aided by Home Care in Jersey

We believe that complex needs should never mean reduced choice or loss of identity. With the right professional support, many people can continue to live fulfilling lives in familiar surroundings, close to family, friends and their community.

Our approach

Complex needs care is never one-size-fits-all. Every individual we support has a fully personalised care plan built around their clinical needs, personal goals and daily routines. Care is delivered by small, consistent teams who understand the importance of trust, continuity and calm, particularly where needs are long term or change over time.

Our teams work closely with families, clinicians and other professionals to ensure care is safe, coordinated and responsive, while always respecting the wishes and preferences of the individual.

Who we support

Our complex needs service supports adults with a wide range of needs, including:

Neurological conditions, acquired brain injury and physical disabilities
• Learning disabilities and autism
• Mental health needs and behaviours that challenge, with support of the Listening Lounge
• Long term conditions requiring skilled or specialist support.

Support can range from structured daily assistance through to 24 hour care, depending on the level of need.

We prioritise consistency of carers wherever possible, as stable relationships are central to effective complex care and positive outcomes.

Skilled teams you can trust

Care is provided by highly trained staff with experience in complex and care. All team members receive specialist training relevant to the needs of the people they support, including positive behaviour support and risk management, and are backed by experienced clinical and operational leadership.

We prioritise consistency of carers wherever possible, as stable relationships are central to effective complex care and positive outcomes.

Independence at the heart of care

Our ethos is simple. We are ambassadors for independence and quality of life. Even where needs are complex, we focus on what individuals can do, not just what they need help with. This may include supporting social activities, maintaining skills, building confidence or helping someone regain independence after a period of illness or hospital admission.

Care is delivered in a way that empowers people, rather than limiting them, and evolves as needs change.

Integrated community support

Our complex needs service is designed to work alongside health and social care services, not replace them. We collaborate closely with GPs, specialist teams, therapists and social workers to ensure care is well coordinated and aligned with wider care plans.

Families are fully involved and supported throughout, with clear communication and a 24 hour on-call service providing reassurance whenever it is needed.

When residential care is the right option

For some individuals, residential care may be the most suitable option when their needs become too complex to manage at home. Our homes offer a safe and supportive environment where residents can receive the specialised care they require.
